Parent/Guardian Consent

I, The legal guardian and/or parent of the above candidate, herby give my consent for his/her participation in the activities, as organzied by the Millville Baseball League.I fully assume all risk, hazards, and medical expenses incidental to such participation, including transportation to and from activities. I, the undersigned, understand that it is a requirement for my child to participate in the baseball and/or softball league’s annual fundraiser, to be determined by the Millville Baseball League’s Board of Trustees. Parents/guardians will be responsible for purchasing game pants and baseball cleats. I understand that should I decide to not honor this agreement, that my child will be removed from his or her team. I do hereby fully waive, release, absolve, indemnify, and agree to hold blameless the Millville Youth Baseball and Softball Association, the Butler County Sportsmen rifle and Pistol Club, sponsors, managers, coaches, Board of Directors, supervisors, umpires, and participants for any claim arising out of injury suffered during any activity or in transport to or from any activity. I do hereby authorize the image and or name of player while participating in any said baseball or softball activity to be used by Millville Youth Baseball and Softball Association for the league’s website. I understand that there will be no refund of registration or fundraiser money for any reason. I do understand that there will be a $35 charge on all returned checks.
